On May 15, 2018, at about 4:40 p.m., two employees were using a forklift to transport a frac tank onto a flatbed trailer. The tank was placed on a metal plate positioned on the forks of the forklift. During transport, the frac tank and metal plate shifted, striking the two employees. One employee sustained two fractured knuckles to the left hand, requiring hospitalization.

On 10/20/2015 at about 11:45 AM, the injured employee was in the process of repositioning forks on a rough terrain forklift with his bare hands (no gloves). His right hand was near the tip end and his left hand at the heel end. He lifted with his right hand so that he could slide the fork along the fork bar. His right hand slipped and his left little finger got caught between the heel and the carriage. The distal end was crushed and severely lacerated (partial amputation).
